1)Heart disease a range of conditions that affect your heart. heart disease is often called "cardiovascular disease", cardiovascular disease generally to conditions that involve narrow or blocked blood vessels that can lead to a heart attack, chest pain and many  more.

2)cardiovascular disease symptoms can cause chest pain, shortness of breath, coldness  in your legs or arms, pain in the neck, jaw, throat, upper abdomen or most likely back.

A very simple webpage with self-explainatory pictures. However it will be better to include some description about the pictures e.g. that under Meaning/Defination.
